If you are a "new client" it is not necessary to fill out the following forms, however, the forms listed below should be used as a guide. If you have any medical conditions, situations, or circumstances referenced in the forms, please discuss them with me before scheduling your massage session.

If you are an "existing client" please share any newly relevant information with me prior to the beginning of your massage session..
